WinShape Homes Requirements for Admission Checklist

STEP I – Intake Process – Once we receive all of the information listed below, WinShape staff will determine if we are the best fit for the child.

  1. Completed WinShape Homes application.  » Application
    (You will need to save the form to your computer after you fill it out and then submit it by email)
  2. Copy of Court Order/Custody Papers (if applicable) [.08(5)(a)5]
  3. Two letters of recommendation.  »  Child Recommendation Form
  4. Most recent Psychological evaluation.
  5. School Records from past two years. (if child is school age) [.08(5)(a)13]
    -Name and Address of Former School
    -Standardized Test Scores – Evaluations
    -Individualized Education Plan (if applicable) – Conduct Reports
    -Special Education/Resource Reports (if applicable)
    -Grades
    -Standardized Test Scores
    -Evaluations

STEP II – Initial Interview

This will include the child, his parents/guardians, and either WinShape Director or a WinShape HSP.  Anyone who is serving as a placing facilitator (family member, counselor, etc.) will also be asked to attend. The staff at WinShape Homes will use this interview as an opportunity to get to know the child and family members in order to determine if WinShape Homes is a beneficial avenue for the child.  This information will be reviewed by the WinShape staff and a decision will be made either to proceed to Step III or to refer the child to a different placement.

STEP III – Team Interview

This interview will include all of those involved in the initial interview plus the Director (if unable to attend the initial  interview) and the House Parents and HSP that WinShape Homes believes will best meet the child’s needs.  The purpose of this interview is to introduce the child and guardian to the House Parents and to all of the WinShape Homes staff that will directly work with the child.

STEP IV – Weekend Visit

WinShape believes that it is imperative to match children with the best possible placement within the program from the very beginning.  With this in mind, a weekend visit helps the child as well as the House Parent determine if that specific WinShape Home is what will best meet that child’s specific needs. After this visit, the final decision on the admittance of a child is made.

STEP VI – Other Documents Needed for Admission
